Learn Your ABCs
Diabetes increases your risk of having a heart attack or stroke, but knowing and keeping your "ABCs" under control can help prevent these and other complications.
- A1c or hemoglobin A1c is a measure of your blood sugar levels over the last three months. Get tested at least twice a year with a goal of keeping it below 7.
- Blood pressure - Aim for below 130/80 to reduce the risk of heart attack and stroke (which is higher among people with diabetes). Get it checked at every doctor’s visit.
- Cholesterol - LDL or "bad" cholesterol can cause buildup in your arteries, which makes your heart have to work harder to pump blood. Get tested at least once a year and aim for an LDL of less than 100, and ideally less than 70.

Diabetes Education & Nutrition Counseling
Hill Physicians provides one-on-one diabetes education and nutrition counseling with a certified diabetes educator or registered dietitian for medical or nutrition-based diabetes education. To receive this benefit, ask your physician or the Member Health Improvement department for a referral. (For Hill Physicians members only.) |
Address Your Stress
Stressful situations can raise your blood sugar and blood pressure in the short-term, and the effects of chronic or frequent stress can lead to diabetes complications. Turn to Hill Physicians’ stress management resources to keep stress in check.
Increase Your Coping Skills
Having diabetes increases your risk for depression. Cognitive-behavioral therapy can help to address negative thoughts and beliefs you might have about either condition.

Hill Physicians' Member Health Improvement department can refer our members to condition-specific disease management programs offered by your health plan. Contact our Member Health Improvement department for more information.
Pediatric Diabetes & Endocrine Nutritional Services
Diabetes education services for children with Type 1 diabetes are available to Hill Physicians members in the Sacramento area. Authorization is required; please contact your child's physician for more information.
Sweet Success Prenatal Diabetes Education Programs
Hill Physicians members who have diabetes and are considering getting pregnant, have diabetes and are already pregnant, or are pregnant and have been diagnosed with gestational diabetes have access to prenatal diabetes education services. Sweet Success care teams of dietitians, nurses, social workers, and physicians will work with you to develop a plan of care that is right for your type of diabetes and pregnancy. To access Sweet Success services, ask your physician for a referral or contact Member Health Improvement. |
