results online. Just register for free RelayHealth service and choose a participating physician, and
you will be able to take care of many routine requests without picking up the phone.

Hill members can create a secure Personal Health Record for 24/7 access to medical information. This makes it
easier to access and update vital health information from anywhere -- home, work or even on the road in an
emergency.

Doctors join together into various medical groups to provide
care. But medical groups are made up of many different kinds of doctors and many different physician
practices.
Think of the medical group as a big team that stands behind your
doctor and supports you in your health. Because each medical group is different, your choice of a group can
impact your care. For example all medical groups:

Checks that its doctors give care that’s been proven to
work.

Sets the rules for you to “get an ok” (a referral) to see
specialists and get other care.

Sometimes decides the steps doctors must follow
to diagnose and treat health problems.

Checks the training and experience of new
doctors.

Decides where you go for hospital care and other
medical services (along with your plan). Provides the network of primary care doctors and specialists that
will take care of you.
Your own doctor probably belongs to one or more medical groups. So,
even when you find the plan and doctor you want, it’s important to choose Hill Physicians if you want to

How Can I Tell My Current Medical Group
If you already have HMO, Medicare Advantage or managed Medi-Cal
coverage, you already have a medical group. Take a look at your membership card. You should see
the name of your doctor and a separate listing for a medical group.
If you don’t see Hill Physicians listed as your medical group, you can easily
switch with any change in your cost or interruption in your insurance coverage.
Available doctors, hospitals and services may vary based on type
of coverage and your home or work location. Discuss hospital preferences with your primary care physician.
