Care Management

​​Hill Physicians is committed to supporting our members at every step of their healthcare journey. Our care management programs are designed to help members dealing with multiple conditions, new diagnoses, serious illnesses or hospitalizations. ​

These programs offer a care team that can include a registered nurse, pharmacist, health educator, social worker and/or other support staff. If you are in one of these programs, your care team will work alongside your doctors, complementing the care they provide and offering additional support as needed.

Care Management

Care Management gives you the support you need to meet your healthcare goals. This includes working over the phone to create and update your care plan, answering any nonemergency health questions between your doctor’s appointments, and connecting you to local services and resources.

Transition Care

Transition Care provides extra support in the time surrounding a hospital stay. In this program, your care team helps you safely transition out of the hospital and avoid complications that could cause you to return.

Serious Illness Management

Serious Illness Management offers assistance and specialized care coordination when you are dealing with a serious or advancing illness. This program focuses on developing a plan that helps manage symptoms and improve quality of life when possible. It also provides support for families and caregivers during stressful times.

Health Plan Disease Management

Our contracted health plans offer disease management programs. Program availability varies by health plan but may include diabetes, asthma, heart disease, smoking cessation, depression, prenatal care, neonatal care and cancer. For more information, contact your health plan.
